Small scaly patch in head

Asked for Male, 35 Years

Small scaly patch in head (two different area) its is almost there for 8 years. Normally infected area dead cells will come out and again the fungus will form. I use nizoral it removes the fungus but its temporary solution Infected area will be black and rough and little elevated from skin
