Dear Sir
Rotator cuff tear usually need shoulder arthroscopy and rotator cuff repair. This surgery is effective provided there is no arthritis. However a detailed examination by an Orthopaedic Shoulder specialist is recommended.
Dear friend
I understand your concern for your grandfather. Shoulder pain with muscle tear if not getting relieved even after exercises and physiotherapy, then definitely surgery is the option best suitable to reduce pain. In this age, usually 2 options are available.
Arthroscopy and Rotator cuff repair or Reverse shoulder replacement if cuff is not repairable. Both these surgeries are costly surgeries and require special skills.

If all these measures have failed then he might need arthroscopic surgery for resolvung the pain. However, the surgery will involve a higher risk due to the age of the patient.
