From last few day 5-6 days, I am feeling extreme pain near by left shoulder and neck region. The pain doesn't seem to reduce and while applying pain relief ointments I observed that there is some swelling or some clot like thing present. On pressing the same the pain increased further. this seems like swelling on the vein or something like that. Can you please advise the reason of this pain and remedy to get relief from the same. Kindly advise a solution for the same.

As per your description of the condition. please answer few more questions . 1) Did you had some Injection on that particular site where you feel sweeling? suggesting of a vascular injury, (also some skin discoloration can also be noticed) , you may use anti-thrombin topical cream so that clot is dissolved (it may take 3-5 Days) 2) Did you hurt your arm like fell on your shoulder. 3) Was there some accidental pull to your arm (as in case of catching a support while falling) Either of the above 2 cases suggests muscular injury. it may be a rupture to rotator cuff. A further detailed assessment will be needed also some investigations may be needed, before we advice you anything.
Next Steps
rule out the type of pain and the injury mode you are having and continue with seeking a consultant and then opting for the appropriate rehabilitation technique

Greetings, sorry to hear about your pain. Going by your complaint it is a muscle spasm and the clot like thing you mentioned is where the spasm is at its most (trigger point).  must massage the area a little harder a couple of strokes and ice the area with raw ice for 5 min to 10 min. Do this twice a day and give rest for couple of days. Along side this you can visit a physio near your place to get this finished. Avoid head bath and two wheel drive for a week or until symptoms reduces to 60%. Wishing you a quick recovery. If any concerns do let me know.

Its most likely that you have sprained your neck. That provokes muscle spasm. Often patients perceive the knotted muscle as a swelling.

You can apply warm compress with a total soaked in warm water to your neck region. This will relax the spam in the muscle. You can take OTC medicines at the pharmacy.

If pain persists beyond 3 days visit a local ortho surgeon.
