Scrotum pain n swelling

Asked for Male, 40 Years

I have little pain on my testicles..pain only when I hit my hand on testicles or I mean small hit or I touched with little force on testicles it pains..normaly no pain...and night time my testicles hanging very very low.i am attaching my ultrasound of scrotum..pls suggest me what to do. I am attaching pic of testicles also..day time size is small of my testicles
