Thank you for your query regarding Fetal Hydronephrosis. Most fetal Hydronephrosis in pregnancy is physiological effect of maternal hormones.
In the third trimester, the definition of Hydronephrosis depends upon the AP diameter of the renal pelvis, calyceal dilatation and parenchymal thickening. AP diameter of more than 7 mm after 30 weeks is considered abnormal. Presence of oligoamnios ( less liquor) and other Congenital malformations along with pelvi-calyceal dilatation suggests uropathy.
It has been seen that most babies with AP diameters less than 7 mm have excellent neonatal outcome. Rest assured that if there is no oligoamnios, your babies urinary tract is okay.
A post delivery ultrasound and measurement of fetal kidneys would be conducted by your neonatologist after communication with your treating Obstetrician, after delivery to rule out obstructive uropathy. At delivery, an AP diameter of 1 cm would be considered clinically relevant for investigation.
