This is my fractured leg , I had fracture and operated my leg 23yrs back. My leg has become red and it feels like there is a boil which has pus in it. (The white/yellow substance). If I rest I am fine but if I work or stand even for sometime, I get very bad burning sensation in that part and itching alot also
I am attaching picture of how it looks and also which cream I use but nothing works. Please suggest.

Do you have diabetes? If yes, your foot/ankle may get infected because of diabetes. Orthopaedic plates/screws can get easily infected if the patient has diabetes.
Even if you don't have diabetes, it is important to find out if you have infection.
Next Steps
Please get a fresh Xray Ankle (AP and lateral views) and fresh Xray Foot (AP and oblique views). Also get the following blood tests done...
1) CBC
2) ESR
3) CRP
Do get back with the fresh Xrays and the X-rays taken after your surgery (if you have them with you).

If there is a rod or plate inside the leg which you have posted please visit an orthopaedic surgeon as soon as possible. the doctor after examination will decide whether it is because of old surgery or any new condition started. and applying the cream u posted will not help .
Next Steps
