Photograph was needed to comment on diagnosis. It can be acne or rosacea. Treatment will depend on diagnosis. In both cases regular treatment only can achieve good control. You have not provided any details of treatment taken in past. I would still advise you to consult a qualified dermatologist for proper diagnosis and treatment. Take care
