Smegma pearls are generally due to prepucial adhesions which generally is inadequate opening of fore skin. You need to apply mometazone cream three to four times daily along with proper hygiene. As the adhesions break and prepuce starts opening they will not reappear.
Next Steps
watch for adequate opening of foreskin. if there is appearance of redness in the area after applying the ointment surgical consultation is recommended.
Health Tips
if itching or redness is there or adhesions do not release surgical consultation is recommended

The white creamy discharge just below the foreskin is called smegma. Just clean it with warm water and cotton balls and maintain hygiene
