Is this due to my sleep cycle or i am possible candidate for implant? My cholesterol stands at borderine and sugar at 97 random, vitamin d at 9 mg/dl

An irregular morning erection alone does not indicate a need for an implant. Sleep pattern, stress and vitamin D deficiency may contribute; improve sleep and lifestyle and consult if erectile problems persist.

Dear Practo User, Morning erections, medically known as nocturnal penile tumescence (NPT), are a normal physiological phenomenon and can occur several times during sleep. Their frequency can naturally vary from day to day and does not necessarily indicate a sexual or medical problem. In your case, sleeping around 4 AM and waking at 11 AM for several years may influence when you experience or notice an erection. Since nocturnal erections are associated with particular stages of sleep, including REM sleep, you may not always notice an erection immediately after waking. The fact that you occasionally experience an erection when getting up to urinate is reassuring and suggests that your erectile mechanism is functioning. Having fewer or irregular morning erections by itself does not mean that you have erectile dysfunction (ED). ED is generally assessed based on your ability to achieve and maintain a satisfactory erection during sexual activity, along with your medical history and other risk factors. Regarding a penile implant, there is no indication from the information provided that you would require an implant. Penile implants are generally considered only in selected patients with persistent, significant ED when appropriate medical treatment and other options have not provided satisfactory results. Your reported Vitamin D level of 9 ng/mL is significantly low and should be discussed with a qualified doctor for appropriate evaluation and supplementation. Your reported random blood sugar of 97 mg/dL is not, by itself, suggestive of high blood sugar. Borderline cholesterol, however, is worth monitoring because cardiovascular health and erectile function can be closely related. Your masturbation frequency of approximately five times per week does not, by itself, establish a cause for reduced morning erections or ED. There is therefore no need to panic based solely on the change in morning erections. A detailed consultation can help assess your sleep pattern, erections during sexual activity, blood pressure, cholesterol, blood sugar, hormonal factors and other relevant health parameters before deciding whether any treatment is required. Your sleep pattern can certainly be contributing to the irregular morning erections. Morning erections are actually related to erections occurring during sleep, particularly around REM sleep, and you may simply notice one when you wake at a particular point in your sleep cycle. The fact that you occasionally notice a good erection around 6–7 AM is reassuring and indicates that your erectile mechanism is capable of producing an erection. Masturbating about 5 times a week does not by itself mean that you have erectile dysfunction. Your borderline cholesterol and vitamin D deficiency are also worth addressing as part of your overall health, since metabolic factors and sleep problems can contribute to erectile difficulties.
Next Steps
You are not a candidate for a penile implant based on this history. Implants are generally considered for established, persistent erectile dysfunction when other appropriate treatments have failed or cannot be used; they are not indicated simply because morning erections are irregular. Your first priority should be to gradually correct the sleep schedule and aim for consistent, adequate sleep rather than monitoring morning erections every day. If you have difficulty achieving or maintaining a sufficiently firm erection during sexual activity despite improving your sleep and lifestyle, then a personalised evaluation would be appropriate. Health Tips
Try gradually shifting your bedtime earlier rather than making an abrupt change—for example, move it earlier by 30–60 minutes every few days and maintain a consistent wake-up time. Get regular daytime exercise and morning daylight exposure, limit late-night screen use and caffeine, and aim for 7–9 hours of quality sleep. You don't need to completely stop masturbation to restore morning erections, although reducing it temporarily is reasonable if you feel it has become habitual or is interfering with your sleep. Most importantly, don't use the presence or absence of morning wood on a particular day as a test of your sexual health; erections naturally fluctuate throughout the night. Thank you
