Your HbA1c of 6.3% is well controlled, but your doctor likely changed from Metformin to Januvia (Sitagliptin) because of your low body weight (53 kg) and to avoid further gastrointestinal side effects or weight loss that metformin can sometimes cause.
Januvia is kidney-safe at your creatinine level (1.0) and helps maintain sugar control without affecting weight.
The cost is higher, but clinically it may be a better fit for your current weight profile
Next Steps
Continue the new medicine as advised for at least 2–3 months and recheck HbA1c
Monitor for any symptoms like low appetite, weakness, or weight loss.
Discuss with your doctor if cost is a major issue—sometimes dose adjustment or switching to a combination tablet can reduce expense.
Health Tips
Maintain regular meals and avoid skipping breakfast.
• Keep monitoring BP since you are on Olmesar 20.
• If you feel your sugar rises on Januvia alone, inform your doctor for modification.

It’s good medicine and it’s best to discuss with your physician. Diabetes is under control with medication. But you can always discuss with your doctor if you find medicine expensive
