Low Sodium level 116mmol/L
My father's sodium level is 116mmol/L and chloride 86.1. He is also suffering from High BP and diabetes. There are no specific symptoms but he does feel weak at times. Please advice.

Sodium levels of 116 mmol/L with other comorbities is not a good sign. He should be hospitalized and screened for any other underlying issues. He needs intravenous Saline for correction of the levels.
Next Steps
Get him hospitalised to avoid further complications
