Leg wound diabetic patient
Hello Doctor, My husband is diabetic, but his sugar levels are currently normal. For the past 2 days, he has been having pain in his leg/foot. Today we noticed a small painful red spot with a dark center on his foot (photo attached). There is pain in that area. We are worried because he is diabetic. Could you please advise what we should do? Does he need to come for a check-up immediately?

Small painful red spot with dark center on the foot in a diabetic patient can be an early infected wound/diabetic foot lesion.

What to do now: • Keep the area clean and dry • Wash with saline and apply antibiotic ointment • Avoid walking barefoot & reduce pressure on the area • Wear soft footwear • Paracetamol for pain if needed • Monitor blood sugar strictly

The presence of normal blood sugars does not rule out that he may not develop the complications of diabets milletus. Please do HbA1 c tested . Is there any history of trauma to the foot.this might  be a diabetic foot if it is a diabetic foot there will be loss of sensations in the foot.

In a diabetic person, a: • Small painful red spot • With a dark center • Associated with localized pain Most commonly suggests: ✔ Early infected boil ✔ Insect bite with inflammation ✔ Small abscess formation ✔ Early diabetic foot infection The dark center may represent: • Pus point • Local tissue necrosis • Clotted blood under skin Even if sugar levels are currently normal, diabetes increases: • Risk of infection • Delayed healing • Risk of deeper spread The fact that it is painful is important — pain usually means inflammation is active. This is not an emergency right now, but it should NOT be ignored.

Immediate Home Care (Today) ✔ Do NOT squeeze it ✔ Do NOT puncture it ✔ Wash gently with mild soap and water ✔ Apply Betadine (povidone iodine) ✔ Keep it dry and clean ✔ Avoid pressure on that foot Check: • Is there swelling spreading? • Is redness increasing? • Is there fever? • Is there pus discharge? Right now there is no need for any checkup There is mild erythema only with a little bit of dead tissue in the centre It can heal with some antibiotic along with anti-inflammatory drugs

It takes more time for wound to heal.But having pain is a good sign because it indicates that nerve sensations are intact because it will be lost in uncontrolled diabetes.Need more evaluation,kindly visit a diabetologist

In diabetes, wound take more time to heal because blood vessels is stiff and hard. So its take time to heal.in your case there is no open pus and open wound .thats why you need to proper care of this.consult for further management.

It looks like a small infected spot or pressure sore. In diabetics, even small wounds need early care.

Do the following, 1. X ray of the foot to make sure no foreign body inside the tissue (this can come as a surprise when found) 2. Take Daily photos of the area 3. Antibiotic - Augumentin 625mg capsules three times a day for a week 4. If he develops fever or the redness is spreading, will need to be seen at the clinic or local hospital for IV antibiotics and review to make sure no abscess is developing. All the best. Please share details of his diabetes, does he have numbness in that part? Please get cbc done and Hba1c done and share results. Till then start tab cefixime 200mg one morning and one evening for 5days  days, tab zerodol sp one every 8hours for 5 days. Clean the wound with some antiseptic solution and apply betadine ointment over it. Consult a gen surgeon physically for  clinical evaluation.

Hello dear, As i can clearly see that the patient is having a red spot on his foot and he is diabetic. Can you plz tell me how long he is diabetic? Is he on oral medication or on insulin? The  redness on his foot after any foot injury or does he wear tight shoe?
