Blood cancer is a broad term. It can be either acute leukemia (acute lymphoblastic or myeloid) or chronic leukemia ( myeloid or lymphocytic). Considering age and history of 4 years, I am suspecting that the patient might be having chronic lymphocytic leukemia (CLL). Treatment of CLL is required only when there is B symptoms (fever or weight loss), bulky lymph nodal masses or Cytopenia.
CART therapy is an attractive option in certain types of blood cancers like relapsed refractory acute lymphoblastic leukemia or multiple myeloma. We will need more information to guide further treatment in your patient
