I understand the difficult times your family and you must be going through. In the given scenario it seems that the situation continues to be same despite efforts of family and friends. The seeming irresponsible behaviour, alcohol intake, dysfunctional relationship, and self harm point towards some psychological or psychiatric ailment. In my opinion your brother and your family will be able to understand the situation better by a psychiatrist or psychological consultation. A comprehensive assessment will be followed by remediation strategies. Hope this helps you. My advice will be to not delay the consultation and seek early help.
