Idiopathic hypertension
72 years old male is aknown case of idiopathic hypertension for about 10 years, he was at the begining on amilodipin 5 mg for about 3 years and BP was controlled but later he developed bilateral leg oedema as a side effect of amilodipin,amilodipin changed to valsartan 160 mg and hidrochlorthiazide 12.5 mg and BP was controlled untill before about 1 month ago which the BP started to rise to about160-170/90-100, there is no any sign and symptom and lab.Tests all normal except s. Creatin 1.3,ESR 35

Metoprolol 25mg od  if controlled  good if not  after 10-14days dose hiked to 50mg od
