As I read the problem, you require medical fitness certificate which I understand is mandatory in some organisation at time of joining.
So it's good if you could visit a doctor or any local dispensary and get it prepared in person.
The doctor would like to examine you and the issue a certificate.
You can also seek help from your family physician.

U can visit near by govt hospital or consult with nearest clinic

No. You can get medical fitness certificate only after clinical examination.
Visit nearest regiatered medical practitioner may be your family doctor. He will first examine you and then give the certificate accordingly.
