Filling or RCT
Aa per the attached x-ray, please let me know whether refilling will do or should it need RCT. Previous filling for the same was done mirw than 10 years ago so I want to check for it, as of today there is no pain in it.

X rays are most widely used diagnostics aid to evaluate dental problems such as a decay in teeth,in this case here deep proximal caries is visible which might be approaching to the nerves of the tooth(pulp). Also x ray is a 2-dimensional image of a 3- dimensional object(tooth) hence to diagnose a caries is arbitary.So you need a visit a dentist to better evaluate on the basis of signs and extent of caries.

Hi, Since there is no pain, I would suggest a conservative approach i.e to clean the decay and restore it. Although, it is close to the nerve and in case we reach it, RCT would be recommended. Having said that, It is important to see it clinically.

Dear Practo User, Radiographic image provided by you contains free hand overlays. A image without that would be very helpful in giving provisional diagnosis. As per the description provided by you and whatsoever I can see in the image I would suggest a conservative approach first. Caries present beneath the filling can be removed slowly and a restoration can be done if the pulp is not exposed during removal of carious tooth structure. By any chance if the pulp is exposed, then you have to go for RCT. Regards Arham Dental Care Delhi

The x-ray is depicting that the decay is very close to pulp, but not reaching the pulp. In such cases when we start cleaning the decay the pulp is generally reached because x-rays are not 100% accurate , so it's better that you get RCT done, You can also take a chance of filling but chances are very bleek of it's success.

if there is no pain you can go for filling with a base or a direct pulp capping

Hi..Though there is no pain ( because it is not infected yet). It is a case of root canal. Cavity is deep and spreading below the filling. Sometimes root canal decision is taken based on depth of cavity. Regards

Well you can go for normal filling with Calcium hydroxide or GIC base because I can not see any Periodontal ligament inflammation in X-ray previous restoration is close to pulp but it is not involving pulp moreover you said you don't have any pain.

Interproximal caries is very close to pulp. And actual extent of caries is more than what is visible on radiograph. So its mostly case of root canal treatment.

Hello Mam, If you are looking for a long term solution,go for the Root canal treatment followed by crown and if you want to take a chance,if the pulp is not exposed during excavation of the caries,then a filling may be tried. Regards Dr Sumanmakam

Its better to get Root canal done and stay away from pain for life, but place a crown after root canal...

Diagnosis is generally established based on clinical and radiographic parameters.However,the Xray indicates RCT and the sooner you go for it,better it is.
