Dear Doctor I.have extreme pain in my neck and shoulder from 2 years.And i cant turn and sleep left side in night.

Greetings,  sorry about the pain.  You definitely are suffering from muscle imbalances at neck and upper back relating to poor posture. Your muscles at neck are over working, hence the pain.
Next Steps
get treated by a physio with knowledge about trigger points, massage and posture correction and even further hire a physio as your trainer for 6 months or so and in 4 weeks time you will see results.  if your are a professional do change ergonomics at work place or if homemaker do change ergonomics at home.
