Hello doctor,
My sister is 8th month pregnant but she is scared of labour pains, her doctor suggested her to go for epuderal but we heard that epuderal has side effect as back pain in long term, is it true kindly confirm.

It's not true. Epidural if properly given don't cause back pain. However due to pregnancy itself patients might have back pain which can be overcome by postpartum exercise.
