In the month of march , my blood sugar in 371 after lunch. after two months medicine, it is reduce to 98, i want to prepare log sheet. how to prepare it, any format, please

Record it in an exel sheet .before meals n after 2 hrs f meals .do an A1c every 3 months n lipid profile n creatinine ,sgot sgpt every 6 months. that should be a sufficient data for a newlly diagnosed diabetic .
