Please get a sonomammography done and consult a Gynaecologist or a General Surgeon for a physical examination. By not following the advice in spite of having symptoms you are taking undue risk for yourself.

If you have persistent problems you really should repeat the breast ultrasound like your doctor had advised . You will have the opportunity to inform them of your present conditions and provide them with the latest ultrasound so it can be compared with the previous. An examination could also be done . Some solutions can be found best at a doctor's chamber rather than an online platform :)
Next Steps
Visit a gynaec/ general surgeon .
