Stop Cetirizine and take levosiz OD
Take cough syrup (according to type of cough dry or productive)
Continue Udilive (not for long term)
Monitor your BP and pulse
Your symptoms of drowsiness and a fast pulse are very likely due to cetirizine, especially at the dose you are taking. Cetirizine 10 mg three times a day is higher than usually needed and commonly causes sleepiness. Fever itself can also increase pulse rate, so both factors together can explain what you are experiencing.
Paracetamol 500 mg three times daily is generally safe, and Udiliv is not known to cause drowsiness or raise pulse. So the main issue here is not overdose toxicity, but unnecessary high frequency of cetirizine.
Next Steps
1)Reduce cetirizine
Take 10 mg only once daily, preferably at night
Or stop it if you don’t have severe allergy symptoms (sneezing, itching)
2)Continue paracetamol
500 mg every 6–8 hours only if fever or body pain is present
Do not exceed recommended daily dose
3)Udiliv
Continue as prescribed for high bilirubin
Follow up liver function tests as advised by your doctor
4)Monitor symptoms
If fever lasts more than 3 days, or jaundice increases, further evaluation is needed ,,,(viral fever, hepatitis, etc).
Health Tips
Cetirizine is meant once daily, not multiple times ,,more is not better
Drowsiness is a common side effect, especially during daytime use
Avoid driving while feeling sleepy
Drink enough fluids and take proper rest ,,fever itself can cause weakness and fast pulse
If pulse remains persistently high even after stopping cetirizine, or if you have chest discomfort, seek medical care
thank you
You seem to be quite ill for a young person.
Please attend your local hospital. ? Cholangitis ? Acute hepatitis ?Decompensated CLD
All the best.

Your drowsiness is most likely due to cetirizine, which commonly causes sleepiness. The current paracetamol dose is safe, but frequent dosing should be limited due to raised bilirubin / liver involvement. Fever with body pain suggests a viral illness but liver function needs monitoring.
Your symptoms:
• Fever
• Body pain
• Cough
• High bilirubin
• Drowsiness and fast pulse after medicines
Possible explanation:
1. Viral infection is most likely causing:
• Fever
• Body aches
• Cough
• Temporary rise in bilirubin (seen in viral hepatitis or viral illness)
2. Drowsiness is very likely from cetirizine, especially at the prescribed dose.
You were prescribed:
• Paracetamol 500 mg three times daily → safe and standard
• Cetirizine 10 mg three times daily → this is higher than usual dose
• Udiliv (ursodeoxycholic acid) → for liver support
Normally:
• Cetirizine is taken 10 mg once daily, usually at night
• Taking it three times daily can cause:
• Excessive drowsiness
• Fatigue
• Feeling heavy
• Sometimes increased heart rate due to weakness/dehydration
Next Steps
Medicine adjustment (general guidance)
• Paracetamol 500 mg → continue if fever >100°F
• Cetirizine:
• Reduce to 10 mg once at night only
• Udiliv:
• Continue as prescribed if bilirubin is high
Also do this
• Drink plenty of fluids
• Take proper rest
• Eat light, non-oily food
• Avoid alcohol and heavy fatty meals
Health Tips
You should consult a doctor or repeat tests if:
• Fever lasts more than 4–5 days
• Yellowing of eyes increases
• Urine becomes very dark
• Severe weakness or vomiting develops
• Pulse stays consistently above 100 at rest
If not already done, basic tests may be needed:
• LFT (bilirubin, SGOT, SGPT)
• CBC
• Dengue/malaria test (if high fever area)
Your symptoms and side effects are most likely due to the higher cetirizine dose, not a serious reaction. With proper dosing and rest, most viral illnesses and mild bilirubin rises improve within a few days.
If you’d like, an online consultation can help adjust your medicines precisely based on your reports and symptoms.
