Anal fissure/internal hemarroids. Go?

I am patient of anal fissure and internal hemarroids from last 12-13 years. Internal hemarroids not been an issue for me. My fissure gets triggers sometimes, after doing some dietary changes it heal. Couple of times , I have face severe pain also. I checked with doctor, he said you have grade 2 internal hemarroids and chronic fissure. He suggested for a surgery. Last month My fissure got triggered couple of times with a gap of 3 weeks. I think because of straining. Daily, I am consuming 30g of fibre. Fruits ,vegetables seeds, dry fruits soaked, water also. Also, my main issue is to clear my stomach, I have to go poop 2-3 times, which is impacting my lifestyle so much. Am i suffering from IBS? How can i clear my stomach in one go? I think because of using washroom multiple times my fissure is triggering. Can you please advice
