Left testacle grows in size

My left testacle grows in size after bowel movements, exercise, or masturbation. I have seen a doctor and he thought it could be emphydemis and was put on antibiotics for a week (doxycycline) and that did not work. Doctor felt for a hernia and could not find one also getting a ct scan they found no hernia. Was tested for all stds and came up clean. Any idea what my problem could be?
