I would advise you to go ahead with the operation rather than having an anatomical malformation which would restrict her movement and cause other problems. Since she is of 6 years of age being sacred to go ahead with the procedure is understandable, constantly reassure her and support her. The anesthesia team may gave her a general anesthesia than giving a block or local as she would be frightened otherwise and the procedure can be done. Post operatively she will most likely have a cast on her hand which will make managing her much easier and you wouldn't require drastic measures of tying her up. One means of making the child comply is by wrapping her body in a bed sheet with the required limb left out so she won't move, apart from which there are other means to go ahead with it. Don't be frustrated, treating children is harder but very much doable. I hope this helps and i wish your sister luck for her operation.
