your symptoms suggest anxiety is more.
first you start with relaxation for 2 hours . do not take stress. do exercise like running for 30 minutes.
do deep breathing exercise for 10 minutes whenever you feel anxious.
as your symptoms are more consult psychiatrist . they will give medicine for 10 - 15 days . it will give good relief
