It could be something called as Unknown Primary with neck node. Many times patients develop neck swelling without having visible or clinical cancer at any site of the body. In such a case thorough investigation needs to be done to detect the site of the cancer. That is why a PET scan is advised to detect the origin of the primary. If still not found then maybe you should undergo panendoscopy to detect any other site of origin. Accordingly the treatment can be planned. I hope this helps you and guides you. Good luck.
