The possible reasons for swelling and pain could be injury, infection or non-infective arthritis. If the fluid which was taken out was sent for lab analysis, It would have confirmed any infection. If he has had infection then short course of antibiotics would have controlled the immediate infection. There may be some persistent infection which could be causing the swelling. If he still has swelling, then taking out the fluid with a syringe and sending it for culture will show if there is any evidence of infection. He would not need an anaesthetic for that. Other blood tests will show if there is any evidence of non infective arthritis like JCA, RA rtf.
