2 months, SOB, unable to eat
Hx esophagitis, hiatal hernia, 8 years. Was on Razo D, later Pan L. 2 months ago, started getting SOB after eating. Can eat only half stomach. SOB became bad couldn't speak or walk inside home. Consulted pulmonologist. Chest x-ray, CT, and PFT normal. Eosinophil count normal. on Montek LC/Duonase spray for allerg. Hypertension 3 months, under control. ECG, 2D echo normal. Aug 10, endoscopy found Lax LES. Esophageal manometry found ineffective esophageal motility (30%). Doc put me on Nexpro 40, Cintapro, Liofen. Weight loss of 5 kg. Currently, severe tightness in chest. Tightness in neck. Pain over ribcage, xiphoid, neck. Numbness in arms and legs. SOB after eating, SOB with any activity. Last few days, unable to lie down in bed. Unable to breathe when lying down. Sleeping only 3-4 hours in sitting posture. Can lax LES, GERD, gastritis can be this bad?

Yes. Lax LES, Hiatus hernia can cause atypical symptoms with severe shortness of breath. You need to evaluate the disease properly. In case of severe reflux and if 24 ph monitoring showing symptoms related to reflux - you might need to undergo surgery (Fundoplication) for relief as you are already on medications.
