"High fever (103.8°F) with cough
Hello Doctor, my daughter is 1 year 10 months old. She developed a high fever today, reaching 103.8°F. She also has a mild cough, a hoarse/changed voice, and noisy breathing while sleeping. Today she was examined by another pediatrician, who diagnosed a lower respiratory tract infection and started amoxicillin-clavulanate (Bax CV), a cough syrup, and Meftal-P. Her oxygen saturation was 98–99%. After Meftal-P, the fever came down, but after about 3–4 hours it rose again to 103.8°F. She vomited once with a lot of phlegm but has not vomited again. Is this expected during the first day of antibiotics, or does she need to be examined again or have any change in treatment?

You can continue the antibiotics and other medications as prescribed. Keep a record of temperature, you can use nasoclear saline drops for blocked nose before feeding or while sleep. Maintain hydration, monitor urine output and look for any loose stools, persistent vomiting or if there is increased rate of breathing/breathing difficulty, chest retractions consult paediatrician immediately.

Fever will take 2/3 days to settle. Antibiotics don’t show results on day 1 itself. Kindly continue the treatment. If needed you can do sponging for fever.

fever, cough, hoarseness, and noisy sleeping, diagnosed with a lower respiratory infection and prescribed amoxicillin-clavulanate and Meftal-P.An immediate re-examination is highly recommended, as persistent high fever after anti-pyretics and single-dose vomiting are common on day one of treatment, but the "noisy breathing while sleeping" and "hoarse/changed voice" are atypical for simple lower respiratory tract infections and closely resemble croup or upper airway swelling.Potential alternative conditions that could cause these croup-like signs alongside a fever include acute laryngotracheobronchitis (viral croup), bacterial tracheitis, or a co-existing upper and lower respiratory infection
