Bariatric Surgery: Overview

Bariatric Surgery is a surgical procedure that helps in weight loss by reducing the size of the stomach. By reducing the size of the stomach, it leads to restricting or decreasing the amount of food intake at a time and causes malabsorption of nutrients. Sometimes bariatric surgery may also lead to hormonal changes.

Bariatric Surgery is considered when you have severe obesity, BMI of more than 40, BMI of 35 with conditions such as diabetes, hypertension, joint pains, abnormal cholesterol levels in the blood, sleep apnea, and risk of developing a heart disease etc. Individuals who are unable to lose weight by medications or keep gaining weight even after medication treatment, changes in the diet intake, and modifications in lifestyle can prefer bariatric surgery.

There are four types of methods involved in Bariatric Surgery to reduce the size of the stomach to decrease the body weight and they are the following:

  1. Laparoscopic adjustable gastric band
  2. Gastric bypass
  3. Sleeve gastrectomy (gastric sleeve)
  4. Biliopancreatic diversion with duodenal switch

Laparoscopic adjustable gastric band: This is performed as laparoscopic surgery. In this procedure, a small stomach pouch is made by using an adjustable band which makes you feel full after eating a small amount of food and helps in weight loss.

Gastric bypass
: This is also known as Roux-en-Y gastric bypass. In this size of the stomach is reduced that leads to consuming a small amount of food or the small intestine is attached to the lower part of the small intestine which may lead to absorb fewer calories and helps in reducing the body weight.

Sleeve gastrectomy (gastric sleeve): This method is also called sleeve gastrectomy in most of the stomach is stapled inside leaving a banana-shaped section and restricts the intake of food.

Biliopancreatic diversion with duodenal switch: Thi process is more complex than other methods and reduces the body weight faster than other methods.

Steps of Bariatric Surgery

There are four types of surgical procedures in Bariatric Surgery:

  1. Laparoscopic adjustable gastric band
  2. Gastric bypass
  3. Sleeve gastrectomy (gastric sleeve)
  4. Duodenal switch

Laparoscopic adjustable gastric band:

  1. In this process, a ring with an inner inflatable band is placed around the top of the stomach by your surgeon to create a small pouch.
  2. This will help in diet restriction as you will feel full after eating a small quantity of food.
  3. The surgeon may change the size of the pouch by injecting or removing the solution with the help of a small device called a port. This will be placed below your skin.

Gastric bypass:

This process is also called as Roux-en-Y gastric bypass. There are two steps involved in this procedure:

              Step-1:

  1. In this, your stomach is stapled by your surgeon, which may look like a little pouch in the top region.
  2. This will make your stomach smaller in size and allows lesser intake of food.

             Step-2:

  1. In this, the small intestine is cut and attached to the lower part of the pouch. Thus food is bypassed through the stomach and the higher portion of the small intestine. This will allow the body to absorb fewer calories.
  2. The bypassed section is connected far away down to the below part of the small intestine. This procedure will change the gut hormones, gut bacteria, appetite, and metabolism. This process is difficult to reverse.

Sleeve gastrectomy (gastric sleeve):

  1. This process is also called vertical sleeve gastrectomy. In this procedure, most of your stomach is stapled leaving a banana-shaped section inside.
  2. This surgery reduces the intake of food by making you feel fuller. This will also affect gut hormones, gut bacteria, appetite, and metabolism. This process is irreversible.

Duodenal switch: It is also known as Biliopancreatic diversion.

  1. This process is more complex than other types. This has two separate surgeries involved.
  2. The first surgery process is similar to gastric sleeve process.
  3. The second surgery involves redirecting food to bypass through your small intestine.
  4. Then the bypassed section is reattached to the last part of the small intestine. This will help in allowing the digestive juices to mix with your food. This procedure will help to lose weight more when compared with the other three methods.

  1. You are restricted to eat for one or two days after the surgery.
  2. You should follow the specific diet as instructed by your doctor for at least 12 weeks.
  3. Start taking liquids first and then progress to soft food and then slowly to regular food.
  4.  Follow all the restrictions on how much to eat and what to eat and drink.
  5. There should be follow-up visits to your doctor after this surgery to adjust the size of the opening of the band or if there any problems caused by the band or if this procedure is not helping you to lose weight. There may be changes seen in the first three to six months of surgery. These changes may be body aches, tiredness, flu-like symptoms, dryness of the skin, feeling cold, hair loss or hair thinning, or mood changes.
